They did at the ward I worked in. I mean, it didn't happen very often, because thin-skins wash out, but almost everybody had somebody who set them off, and who they'd hand off to someone else if at all possible. It's actually better for everyone if the team is open about it among ourselves, since that way we can all cover for each other. She voluntarily chose the field where you have to give patients the most benefit of the doubt for their behavior, the field where you treat them because of their inappropriate behavior rather than in spite of it. And she publicly announced that it is unreasonable for her to have to treat the crazy asshole saying offensive shit. That's like signing up for urology when you find dicks triggering.

Fair enough on the first point, but on the second point, saying the quiet part out loud is rarely healthy. If people quietly cover for each other, great. I mean, we all know that there are probably lots of doctors who chose their field precisely so they'd rarely or never have to deal with certain conditions, but everyone would look at them funny if they issued proclamations on how they hate treating those cases. If a doctor said that they never had it in them to sort out, say, complicated autoimmune disorders, well, fine. If they said that they think some of those people are nuts and faking it (and, FWIW, certain psych disorders do seem to overlap with autoimmune disorders, for one reason or another), well, not good.
After training. During training, maybe it's better to try to avoid developing (or at least announcing) too strong of a "I'm not helping that guy" mentality. Going in I was expecting to "take issue" with what I found there. But what I found there was pretty much on point.
One quibble, "observation 8" needs to be folded into "observation 2". If you have a menu, then YOU HAVE A MENU, but if you don't, you don't. It's not required that you have one.
